In response to requests from our Members, the College has designed a sample contact lens specification form which Members may like to use for their patients. 

The statutory requirement is to give patients a contact lens specifcation which includes information such as the date the fitting was completed. The College feels that it is useful, and in the patient's best interests to include additional information where possible such as the modality of wear. This will help the supplier gauge how many lenses it is in the best interests of the patient to supply before the specification expires.

The details which are required by the GOC's Contact Lens Specification Rules are in black type, the additional details you may like to consider adding to the specification are in blue type.
