New Clinical Management Guideline on idiopathic epiretinal membrane

The Clinical Management Guidelines (CMGs) set out the evidence to inform your clinical practice and support professional judgement with respect to diagnosis and management.

Idiopathic epiretinal membrane (ERM) is a common condition of the posterior pole. With the increasing use of optical coherence tomography (OCT) imaging in optometric practice, a higher number of asymptomatic cases with subtle clinical signs are being detected. We are aware that this has resulted in uncertainty in decision making, particularly in the early stages of disease, with some patients being referred that would not benefit from surgical intervention.

To help streamline decision-making and prevent unnecessary referral, we have produced a CMG on idiopathic ERM, which makes up over 90% of all ERM cases. The CMG will support clinicians to distinguish between cases that can be monitored in practice, and those requiring referral for further investigation and management. The referral criteria in the CMG align with the Royal College of Ophthalmologists concise practice point guidance for ERM. 

We are also working with the Royal College of Ophthalmologists and Macular Society to produce patient information and safety netting advice on idiopathic ERM. This should be available in the coming months.
